【摘要】  目的  通过与传统的大隐静脉高位结扎、浅静脉剥脱术相比，考察大隐静脉曲张激光治疗的可行性及其疗效。方法  选择进口激光治疗仪对我科2003年初至今的43例患者施行激光治疗，选择病例均为大隐静脉主干曲张型且病情较轻者；常规硬膜外麻醉或腰麻，穿刺内踝上方大隐静脉主干，向上插入泥鳅导丝，顺导丝导入导管，撤出导丝交换激光光纤，确认在大隐静脉主干，开启激光，设定好功率，边烧边退，同时助手沿大隐静脉沿途压迫，使静脉腔闭合，全程大约50～80s。烧灼完毕后，弹力绷带加压包扎。术后两天出院，2周后解除弹力绷带，配弹力袜3～6个月。结果  43例患者无短期术后并发症发生，单侧肢体平均手术时间27min，最短仅为12min；术后随访3～6个月，多数效果良好，1例复发，再行大隐静脉高位结扎浅静脉剥脱术而痊愈。结论  大隐静脉的激光治疗对大隐静脉曲张主干型且分支曲张病情较轻者，效果良好，但其远期疗效尚待有进一步观察。

    Miniinvasive operation-laser treatment for great saphenos vein varicosis

    【Abstract】  Objective  Discuss the feasibility and therapeutic effect of endovenous laser treatment(EVLT) for varicose vein of lower extremity by contrasting with traditional high ligation and stripping of the great saphenous vein.Methods  We selected the EVLT to treat 43 patients in our department by using laser treatment instrument.We chose the patients with venous trunk type which are not serious.After epidural anesthesia or lumbar anesthesia we stick through the trunk of great saphenous vein superior the medial malleolus.Insert the guide wire upward，then insert the vessel along the guide wire，then withdraw the guide wire and insert fiber through vessel.First you must onfirm that the laser fiber is in the venous trunk，then turn on the laser，enactment power，pull the fiber and cauterize at the same time while the assistant press the leg along the great saphenous vien to make the lumen of vein occlusion.It maybe last 50～80s.After the operation we u无忧论文【http://www.uklunwen.com】se pressure bandages to make a pressure dressing.At the third day after the operation patient can leave the hospital.Pressure bandages can be taken off after two weeks，then dressing compression socks for 3～6 months.Results   The 43 patients did not have short-term complication after the operation.Average surgery time is 27 minutes in single leg.The shortest is only 12 minutes；After operation we made a follow-up for 3～6 months，The most effects are good，only 1 patient retured，then he recovered after high ligation and stripping.Conclusion   EVLT effertive treatment to patients with venous trunk type and ramus varicose vein is not serious，but its prospecdtive efficacy need further observation.

    下肢静脉曲张是一种常见且多发的疾病，早在公元前古埃及已有记载。多在青壮年发病，逐渐加重，早期表现为肢体的酸胀不适、浅静脉迂曲成团，而常规结扎浅静脉剥脱术，创伤大，术后恢复时间长。我科从美国引进大隐静脉激光治疗仪，从2003年初至今，共治疗大隐静脉曲张患者43例（49条肢体），取得较好疗效，报告如下。

    1  资料与方法

    1.1  一般资料  43例患者，男12例，女31例;年龄30～55岁，平均37.8岁。选择病例均为大隐静脉主干曲张型且分支病情较轻者，术前均经多普勒或下肢静脉顺行造影证实深静脉通畅，且排除血管高凝固状态、高龄、严重皮肤病、营养障碍的患者。  1.2  方法  常规硬膜外麻醉或腰麻，取18号套管针穿刺内踝上方大隐静脉主干（有时需做小切口找到大隐静脉主干），向上插入泥鳅导丝，选取5F透光造影导管，测量穿刺点距腹股沟韧带下方2cm处的距离，剪断导管头端并削尖，顺导丝导入大隐静脉主干，撤出导丝，交换激光光纤，连接激光治疗仪，应在腹股沟区皮下看到光纤头端的指示红灯，反之则为进入深静脉，需退出重新插入；开启激光治疗仪，设定发射功率为12W，脉冲时间为1s，间隔1s，边烧边退，每秒约退1cm左右，同时助手沿大隐静脉行走压迫，使静脉内腔闭合；全程大约需要50～80s；退出导管及激光光纤，弹力绷带加压包扎。

    术后常规应用抗生素，2天以后可以出院，改为口服抗生素。2周以后，解除弹力绷带，配弹力袜3～6个月。

    2  结果

    与同期常规大隐静脉主干手术组相比，激光治疗组的手术时间、住院天数、止痛剂应用例数及抗生
